The American Dental Association has updated their claim form for 2024. Effective 1/1/24 this should be the only form being used and the 2019 will be discontinued. Attached is an image with the instructions from the ADA. New inventory will be on the shelf in November. Same price as current forms. Image of just the form from the ADA is also attached.
